Job Descriptions

Our staff meet and exceed State of Florida requirements and follow Federal Medicare standards

to ensure client comfort, care and satisfaction.  Some of these essential requirements include:
 

Case Manager (CM)
Our home health clients will be provided with a Case Manager, who is a Registered Nurse.  The Case Manager will assess and design a personalized care plan for each of our clients including treatments, education, medication review, nutrition and safety evaluations, and assessments for nursing home placement.  The Case Manager also works with family members, physicians and other professionals involved with the client's health care.

Registered Nurse (RN)
They provide direct hands on skilled nursing assistance for the administration of medication and treatments for the patient.  Including assessment, planning and implementation of care, advanced treatments and procedures, and direct care when necessary to instruct Home Health Aides, Certified Nursing Assistants, and Licensed Practical Nurses. They also assist and maintain quality of care throughout our services and monitor the status of patient care, report and take orders from the Physician in charge of care.


Licensed Practical Nurse (LPN)
 A LPN can instruct Home Health Aides, Certified Nursing Assistants, and provide direct hands on skilled nursing assistance for the administration of medication, tube feeding, wound care and treatments for the patient.  An LPN can also monitor the status of patient care and report and take orders from the Physician in charge of care. They also provide other necessary care such as bathing, dressing, feeding, range of motion etc.

Home Health Aids (HHA) / Certified Nurse Aide (CNA)
Certified HHA/CNA have the minimum 75 hour additional training as per the Medicare standards.  They are skills tested and carefully screened prior to being employed.  They provide this level of care:

Limitations: These Caregivers cannot administer medications of any kind although they can remind the patient to take medications and home exercise programs under the direct supervision of the professional nurse or physical therapist.

 Physical Therapy (RPT)
Registered Physical Therapists provide assessment, planning and implementation of care, gait training, transfer training, and therapeutic exercise programs for the functionally impaired.

Occupational Therapy (OT)
Licensed Occupational Therapists provide services to enable patients to regain independence in performing activities of daily living, personal care, and homemaking.

Speech Therapy (ST)
Licensed Speech Therapists provide a progressive rehabilitation program for patients experiencing difficulty in speaking, swallowing, or       communicating.

 Masters of Social Work (MSW)
 
Certified Masters of Social Workers provide information and referrals to community resources, assistance with nursing home or assisted living placement, application  for Medicaid or other benefit programs, and counseling for the patient and family.

 

 Nutritional and Respiratory Therapists are provided as prescribed.

 As with all our services they are delivered right to a client's doorstep.
